Athletic Clearance Information

1. District Forms:  Miramonte and the Acalanes Union High School District require various forms be completed. If your student has not participated in a high school sport this year, please go to the link below to complete the on-line registration:
   Online Registration & Athletic Clearance
2. Print and Complete  the Athletic Clearance Form (requires a physical exam and physician’s signature)
3. Print the Online Registration Sports Summary Form.
4. Bring the above forms to Athletic Director, Coach Dell’ Aquila to become eligible to participate in the swim team.
5. Pay the District Spring Sports Participation Fee and Swim Team Spirit Fee through the MHS Webstore.

If your student has participated in a sport in the Fall and/or Winter they must still submit a copy of the Online Registration Sports Summary form* to the Athletic Director and  pay the District Spring Sports Participation Fee and Swim Team Spirit Fee through the MHS Webstore.
*to print out this form you need to go through the online registration process again.
